Etoricoxib
Indications/Uses
Symptomatic relief of OA, RA, ankylosing spondylitis, & the pain & signs of inflammation associated w/ acute gouty arthritis. Short-term treatment of moderate pain associated w/ dental surgery.
Dosage/Direction for Use
Adult & adolescent ≥16 yr OA 30 mg once daily. May be increased to 60 mg once daily in patients w/ insufficient relief from symptoms. RA 60 mg once daily. May be increased to 90 mg once daily in patients w/ insufficient relief from symptoms. Once patient is clinically stabilized, down-titration to 60 mg once daily may be appropriate. Ankylosing spondylitis 60 mg once daily. May be increased to 90 mg once daily in patients w/ insufficient relief from symptoms. Once patient is clinically stabilized, down-titration to 60 mg once daily may be appropriate. Acute pain conditions Should only be used for acute symptomatic period. Acute gouty arthritis 120 mg once daily. Post-op dental surgery pain 90 mg once daily for a max of 3 days. Mild hepatic dysfunction (Child-Pugh score 5-6) Max: 60 mg once daily. Moderate hepatic dysfunction (Child-Pugh score 7-9) Max: 30 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Severe heart failure. CABG surgery. GI bleeding & peptic ulcer. Pregnancy (3rd trimester).
Special Precautions
CV & GI risk. Renal effects. Advanced renal disease. Serious liver injury. Pregnancy (3rd trimester). Lactation. Elderly.
Adverse Reactions
Alveolar osteitis; edema/fluid retention; dizziness, headache; palpitations, arrhythmia; HTN; bronchospasm; constipation, flatulence, gastritis, heartburn/acid reflux, diarrhea, dyspepsia/epigastric discomfort, nausea, vomiting, esophagitis, oral ulcer; increased ALT & AST; ecchymosis; asthenia/fatigue, flu-like disease.
Drug Interactions
Oral anticoagulants; diuretics, ACE inhibitors & AIIAs; ASA; cyclosporin & tacrolimus; lithium; methotrexate; OCs; HRT; prednisone/prednisolone; digoxin; effect of etoricoxib on drugs metabolized by sulfotransferases; effect of etoricoxib on drugs metabolized by CYP isoenzymes; effects of other drugs on the pharmacokinetics of etoricoxib; ketoconazole; voriconazole & miconazole; rifampicin; antacids.
